First-time Patient Information
Patients being seen for the first time at this hospital
Please fill out the necessary information on a “Medical Record Form” at either the desk located near the main entrance and in front of the counter. Bring your completed form, insurance card, referral, and any other necessary items to the “New Patient Reception (General Reception)” counter located in the 1st floor outpatient hall to complete your registration.
Reception hours are from 8:30 a.m. – 10:30 a.m.
After completing registration with reception you will receive a patient registration card, a hospital pager, etc. Please present everything except the hospital pager to the reception desk of the department where you wish to be seen.
Please keep the hospital pager with you until you settle your account at the end of your visit.
Patients being seen for the first time at a different department than a previous visit
Please take your patient registration card, insurance card, etc. to the “Returning Patient Reception/Other Department New Patient Reception (General Reception) counter located in the 1st floor outpatient hall and complete the reception registration.
Reception hours are from 8:30 a.m. – 10:30 a.m.
After completing registration with reception you will receive a patient registration card, a hospital pager, etc. Please present everything except the hospital pager to the reception counter at the department where you wish to be seen.
Please keep the hospital pager with you until you settle your account at the end of your visit.
Patients possessing a patient registration card issued prior to June 1999
The same procedure applies as patients who are being seen for the first time by a different department than a previous visit. Please follow the above registration procedure.
